Premenstrual Syndrome ( PMS )- Definition, Types, Diagnosis and Risk Factors


I. Definition
Premenstrual syndrome effects over 70% to 90% of women in the US and less for women in Southeast Asia because of their difference in living style and social structure. Since it was classified as a syndrome, the occurrence have more than doubled over the past 50 years. Premenstrual syndrome is defined as a condition of faulty function of the ovaries related to the women's menstrual cycle, it effects a women's physical and emotional state, and sometimes interferes with daily activities as a result of hormone fluctuation. The syndrome occurs one to two weeks before menstruation and then declines when the period starts. In some women, the symptoms are so severe, they have to take time off work and restrict themselves from attending normal daily activities.

II. Types of premenstrual syndrome
1. Anxiety
Anxiety is a common symptoms for women with PMS, it is caused by hormone imbalance between estrogen and progesterone. Most women with PMS is found to very high levels of estrogen and low levels of progesterone before menstruation.

2. Craving
Some women with PMS may experience the symptom of food craving, it is caused by abnormal liver function in carbohydrate synthesis. Without coordinating between liver and pancreas, it causes high levels of insulin in the blood stream, resulting in increasing the risk of sugar craving. Other theory suggest when a woman under stress before period, she may grasp for everything which makes her feel more comfortable.

3. Hyperhydration
Breast tenderness or water retention are always a problem for some women with PMS. Breast tenderness is caused by over production of prolactin as resulting of abnormal function of pituitary gland. Water retention is either caused by inability of digestive system in absorbing of potassium or abnormal function of lymphatic function. Some theories also suggest that fluid retention is caused by high levels of serotonin.

4. Depression
Researchers found that women with symptom of depression always have unbalance levels of estrogen and progesterone as resulting of liver abnormal function of fat and protein metabolism. Without right levels of estrogen and progesterone, it causes nervous tension resulting in increasing nervous disorder including depression.


III. Diagnosis
Premenstrual syndrome may be similar to other types of symptoms such as candida, diabetic reaction, allergic intolerance, thyroid function. No test can diagnose PMS, the only method is to photocopy and fill in the menstrual symptom diary. If there are increasing symptoms in the two weeks before menstruation then it must be premenstrual syndrome, otherwise you can rule out PMS.

IV. Risk factors
Researchers found that women with the below categories will be more likely to develop the premenstrual syndrome:
1. Genetics
If any one in your family have it, you may have it, although there is no proof about it.

2. Age
If you are between 30 - 40 years of age

3. Children
If you have more than 2 children, your risk of premenstrual syndrome increases significantly.

4. Pregnancy succession
Your risk of PMS increases if you have experienced many pregnancies in quickly succession.

5. Hormone Upheaval
If you have experienced a hormone upheaval caused by miscarriage, pregnancy, or pregnancy termination, you are at a high risk of developing PMS.

Chapter II

A. Causes
1. Unhealthy diet
Over consumption of saturated fat, trans fat, artificial, caffeine, etc and under consumption of fiber, vital vitamins and minerals are typical diet in our country resulting in over production of bad estrogen leading to weakened the liver function in carbohydrate synthesis and fat and protein metabolism causing PMS.

2. Digestive system
Strong digestive system is important to provide vital nutrients for our body in regulating its normal function. Unfortunately, it is unknown that women with PMS are found to have digestive disorder before menstruation resulting in symptoms of PMS.


3. Vitamins and Minerals deficiency
Vitamins and minerals are important for regulating the hormone production in our body, especially for women during menstrual cycle. Deficiency of vitamins and minerals causes nervous disorder, liver malfunction and hormone imbalance resulting in symptoms of PMS.


4. Sluggish liver
For what ever reason, women with PMS are found to have a sluggish liver before menstruation. Since liver is vital in regulating the levels of estrogen through production of cholesterol and fat and protein metabolism, sluggish liver causes nervous tension and hormone imbalance.

5. Hormonal imbalance
Over production or under production of certain hormones before menstruation as resulting of thyroid malfunction, pituitary gland abnormality function, liver inability in metabolism, abnormal function of adrenal gland etc. cause hormonal imbalance resulting in symptoms of PMS such as depression, stress, insomnia, fatigue, tiredness, menstrual cramps, etc.

B. Symptoms
1. Emotional symptoms
a) Unhealthy diet
Since we all know that our diet contains high amount of saturated and trans fat, unbalance of levels of essential fatty acids causes inability of liver in secreting bile into the digestive system and in fat and protein metabolism that causes nervous tension resulting in increasing the emotional symptoms of PMS such as stress, anxiety, tiredness, insomnia, etc.

b) Vitamins and minerals deficiency
It is unknown why women with PMS are found to have low levels of vitamin B complex and magnesium. These may be caused by low levels of stomach acid or inability of digestive system in absorbing vital nutrients before period resulting in increasing nervous tension and emotional and physical symptoms.

c) Hormone imbalance
Researchers found that women with PMS always have unbalance of levels of estrogen and progesterone before menstruation. It may be caused abnormal function of some glands in the body including the pituitary, thyroid gland.
Remember over or under production of certain hormones such as serotonin, estrogen, progesterone, prolactin, adrenaline increases the nervous disorder resulting in symptoms of PMS.

d) Thyroid gland
Women with PMS are found to have low thyroid function resulting in increasing the risk of depression, stress, fatigue, tiredness, insomnia, etc.

2. Physical symptoms
a) Foods and sugar craving
Strong liver is important for women with PMS. Unfortunately, most women with PMS are found to have sluggish liver function before menstruation caused by abnormal function of liver in carbohydrate synthesis and insulin regulating resulting in foods and sugar craving.

b) Breast tenderness and water retention
For what ever reasons, researchers show that some women with PMS are found to have high levels of prolactin and low levels of potassium as resulting in abnormal function of pituitary gland and lymphatic function resulting in breast tenderness and water retention accordingly.

c) Palpitation
Some women with PMS might experience heart palpitation, it is caused by malfunction of thyroid gland, unbalance of estrogen and progesterone and inability of liver in secreting bile into digestive system.

d) Nervous tension
Our nervous system needs the strong liver in fat and protein metabolism to function normally and the right levels of estrogen and progesterone to transmit information between cells. Sluggish liver and unbalance of estrogen and progesterone cause nervous tension resulting in symptoms of PMS.

c) Menstrual cramps
Some women may experience menstrual cramps which will disappear as soon as period start. It may be caused by irregular function of ovaries or essential fatty acids deficiency resulting in estrogen and progesterone imbalance and over production of certain hormone in the prostaglandins family.

Premenstrual Syndrome (PMS)- Types of Premenstrual Syndrome

A. Anxiety
1. Hormone imbalance
For what ever reason, women with PMS are found to have high levels of estrogen and low levels of progesterone. It may be caused unbalance of omega fatty acid, deficiency of certain minerals and vitamins, digestive disorder in nutrients absorption, etc, resulting in increasing symptoms of anxiety of PMS.

2. Sluggish liver
Besides helping to detoxify the body and fighting against invasion of foreign substances such as virus, bacteria and abnormal cell growth, liver play an important role in providing nutrients to the nervous system in fat and protein metabolism. Unfortunately, most women with PMS are found to have a sluggish liver during menstrual cycle resulting in nervous tension leading to anxiety.

3. Nutrients deficiency
Deficiency of Omega 3 fatty acid or imbalance of fatty acid in the body during menstrual cycle as resulting of over consumption of saturated fat caused by interference of liver in fat and protein metabolism resulting in increasing tension of nervous system leading to anxiety.

4. Genetic passing through
If your sister has it, you may also have it. For what ever reason, anxiety during menstrual cycle seems to run in the family. It may be caused by high levels of estrogen in the pool gene or reaction of certain glands in production of hormone during menstrual cycle.

5. Environment toxins
Over exposure to toxic environment resulting in accumulation of xenoestrogen in the body during menstrual will lead to over production of bad estrogen and low levels of good estrogen to counter the reaction to the symptoms of PMS.

B. Food Craving
1. Sluggish liver
Maintaining a strong liver is vital for women with PMS, because liver helps in synthesis of carbohydrate and regulating the levels of insulin in the blood stream by transmitting information to pancreas. If the levels of insulin is too high, it tell pancreas to stop producing insulin and if the levels is too low, it tell pancreas to secret more insulin. Unbalancing blood sugar in the blood stream causes Foods and sugar craving.

2. Hormone imbalance
a) Adrenal gland
Women with PMS are found to have high levels of adrenaline which causes less production of progesterone leading to hormone imbalance and resulting in increasing the appetite of sweet and sugar products.

b) Hormone fluctuation
Hormone fluctuation and hormone deficiency, such as over production estrogen and little progesterone or over production of progesterone and less estrogen will increase the tension of nervous system resulting in stress and food craving.

3. Iron and Vitamins B6 deficiency
Some women with PMS during menstrual cycle are found to have low levels of iron and vitamin B6 resulting in increasing the risk of craving for for food not normally eaten and chocolate.

4. High levels of glucose tolerance
For what ever reason, women with PMS are found to have high levels of glucose tolerance such as increased appetite and craving for sugar and chocolate. It may be caused by stress and high levels of adrenaline. Some researchers argue that under stress, most women look for anything which can make them feel comfortable including sweet and chocolate.

5. Omega 3 fatty acid deficiency
Omega 3 fatty acid helps not only to regulate uterus muscle during menstrual cycle, but also helps to balance the levels of prostaglandins hormone resulting in lessening the risk of food and craving for women with PMS

C. Hyper-hydration
1. Lymphatic malfunction
Lymphatic system is important for returning excess interstitial fluid to the blood. Some women with PMS with water retention symptom, it may be caused by lymphatic malfunction leading to over returning of interstitial fluid surrounds the tissue cells resulting in water retention in the body tissues.

2. Over production of prolactin
Prolactin is vital for milk production for feeding mother, over production of prolactin by pituitary gland during menstrual cycle as resulting of hormone imbalance causing breast tenderness for some women with PMS.

3. Digestive system
For what ever reason, women with PMS are found to have digestive disorder. It may be caused by lacking stomach acid in the digestive system resulting in reducing the absorption of certain nutrient which are necessary to maintain right levels of fluid in the body tissues.

4. Potassium deficiency
Potassium is vital for sodium regulation. Without enough levels of potassium, it causes high levels of sodium accumulated in the body resulting in increasing the risk of fluids retention.

5. Serotonin
Serotonin besides plays an important role in neurotransmitter, it also helps to calm the nervous tension with right levels, but over production of serotonin causes nervous tension and water retention.

6. Prostaglandins
Beside helps to regulate the uterus muscle, over production of prostaglandins hormones increase the risk of pre menstrual cramps and water retention.

D. Depression

1. Aldosterone
Aldosterone produced by adrenal gland, besides helping the kidney in regulating water retention in the body tissues by releasing potassium in the kidney, it also acts on the nervous system in controlling anxiety, fear and stress. Women with pre menstrual syndrome are found to have high levels of aldsterone resulting in increasing emotional and physical stress.

2. Glucocorticoids
Glucocorticoids is also produced by adrenal gland, besides helping liver in regulating the glucose metabolism and immune system in fighting against foreign invasion such as bacteria and virus, it also have an negative effect on central nervous system. Over production of glucocorticoids cause stress.

3. Cortisol
Women with pre menstrual syndrome are found to have high levels of cortisol, it is either caused by inability to cope with daily stress, hormone imbalance or nutrients deficiency before menstruation.

4. Serotonin
Serotonin is a monoamine neurotransmitter synthesized in serotonergic neurons in the central nervous system. Unbalancing of levels of serotonin before menstruation causes nervous tension, water retention, anger, aggression and lost of concentration and stress.

5. Androgen
Androgen is a steroid hormone, it helps to stimulates or controls the development and maintenance of masculine in the body. Unbalancing of androgen can influence our behaviour, such as emotional and physical stress, because our nervous system is sensitive to steroid hormones.

Premenstrual Syndrome (PMS) and What to Avoid


A. Artificial Sweetener
1. Is artificial sweetener safe?
We always think that sweetener helps to control weigh and misled in believing that sweetener is better that sugar. In fact, it is not true. Sugar helps to provide energy for our body need, over consumption is the problem while sweetener contains aspartame, the substance causes more harm than good to our body including weigh gain and symptoms of pre menstrual syndrome such as depression and anxiety.

2. Serotonin
Artificial sweetener depresses the nervous system in fighting against stress resulting in less levels of serotonin being produced leading to mood swing and depression.

3. Amino acids
Amino acids are important for liver in protein metabolism. Artificial sweetener contains high levels of aspartic and phenylalanine and it is over 150 times sweeter than sugar, taking artificial sweetener may cause over dose of such amino acids resulting in increasing the risk of nervous tension leading to memory lose, mood swing, depression and symptoms of PMS.

4. Methanol
Artificial sweetener when digest, methanol in the sweetener converts to formaldehyde which is toxic to our body resulting in unbalancing the protein metabolism of the brain leading to symptoms of PMS such as mood swing, anxiety and depression, if it is over dose.

5. Addictive
The chemical aspartame is so addictive. It is wise to stop taking artificial sweetener slowly or ask your doctor for help. Abruptly stop taking aspartame may cause withdrawal symptoms.

B. Caffeine
1. Vitamins and minerals deficiency
Food containing caffeine also contain tannin which block the digestive system in absorbing of vitamins and minerals in the digestive tract including vitamin B6 and magnesium. the vitamin and mineral which are vital for women with PMS.

2. Unbalance of blood sugar
Caffeine is a stimulant, it helps to improve the function of nervous system if it taken in a right dose. Over dose of caffeine causes suddenly surge and fall of blood sugar in the blood stream resulting in over production of adrenaline hormone leading to symptoms of PMS such as mood swing, depression and irritation.

3. Methylxathines
Methylxathines in food containing caffeine besides helps to inhibit the actions of sleepiness inducing adenosine and treat symptoms of asthma , it has been proven to have side effects such as breast tenderness, begnin lumps and breast pain.

4. Theophylline and theobromine
Foods containing caffeine also contains methylxathines and theobromine which besides help to relax muscle and increase blood, if they are taken in small dose. Over dose of methylxathines and theobromine may increase the risk of blood sugar unbalance leading to over production adrenaline hormone resulting in food and sugar craving.

5. Addictive
Caffeine is so additive and it narrows your blood vessels when taken, if you stop it completely and suddenly, you blood vessels will widen causing spitted headache. It is wise to stop it slowly over a period of time.

C. Alcohol
1. Liver functions
Moderate intake of no more than 2 glasses of wine is helpful in liver fat and protein metabolism by increasing the blood flow and awareness of the nervous system. Excessive drinking damages the liver function
a) Detoxifying Liver is a first line defence of our body besides in fighting against foreign invasion such as forming of free radical and bacteria and virus, it also helps to detoxify our body toxic caused environment toxins accumulation or any harmful substance coming into our body through diet or the air we breath, etc. Without strong liver , it causes over production of estrogen resulting in premenstrual syndrome.
b) Essential fatty acids metabolism Without strong liver function, it causes over production of bad prostaglandins hormone PGE2 resulting in increasing the symptoms of PMS including Menstrual pain and cramps.
c) Food craving Liver is essential in regulating the pancreas in production of insulin. Abnormal function of liver in crease the risk of over production of insulin resulting in over production of adrenaline hormone which causes stress and nervous tension.

2. Nutritions and minerals deficiency
Alcohol inhibits the breakdown of vitamins and minerals into usable molecule by decreasing digestive enzymes secretion and damage the lining of stomach in absorbing vital nutrients by blocking the transportation of nutrients into the blood stream resulting in nutrients deficiency including vitamin B complex, zinc, magnesium, potassium etc. leading to symptoms of PMS. Example : Low levels of zinc causes over production of prolaclin resulting in breast tenderness Low levels of potassium causes abnormal lymphatic function and high levels of sodium resulting in fluid retention.

3. Liver disease
Moderate drinking alcohol generally produces feelings of relaxation and cheerfulness, but excessive drinking has the opposite effects as fatty acids build up as plaques in the capillary around liver cells causing liver disease resulting in increasing the severeness of PMS.

D. Dairy Foods
1. Saturated fat
Most dairy food contains high amount of saturated fat which is one form of triglycerides which deceases the function of liver in secreting bile and lessens the function of liver in fat and protein metabolism resulting in lessening the oxygen transportation to the nervous system causing nervous including depression and emotional and physical stress.

2. Magnesium
Dairy foods contain substances which block the digestive in magnesium absorption. Since magnesium is vital for women with PMS, because magnesium deficiency causes nervous tension resulting of anxiety, depression and mood swing.

3. Arachidonic acid (AA)
Diary foods also contains high amount of arachidonic acid which is converted to bad prostaglandins hormone resulting in interference with our body in production of good prostaglandins hormone causing hormone imbalance and menstrual pain and cramps.

4. High levels of sugar
As we mentioned before, high levels of sugar may interfere with normal functions of pancreas resulting in over production of insulin leading to over production of adrenal hormone causing depression, symptoms of PMS. you can avoid this, by choosing diary product with no or little sugar in it.

5. Good bacteria
Some dairy product contains live and good bacteria such as lactobacillus acidphilus and bifidus, they helps to control the production of bad bacteria in our body and increase the immune system in fighting against the forming of free radicals and bacteria and virus thereby, it reduces the risk of symptoms of PMS caused by over production of bad bacteria and immune weakness.

E. Salt
1. Water retention
Increasing intake of sodium (salt) not only reduces the lymphatic function in regulating the fluid in the body tissues and is also harm your kidney in urinary secretion resulting in increasing the risk of water retention.

2. Potassium Sodium in the salt works together with potassium to maintain the fluid in the body. Over intake of sodium disturbs the ratio of potassium and sodium resulting in increasing the fluid retention in the body tissues for some women with PMS.

3. Adrenaline hormone
As the levels of sodium increasing, it not only causes weight gain but also increases the risk of over production of adrenaline hormone as resulting of lowering levels of blood sugar triggering the symptoms of PMS including bloating and puffing.

4. High blood pressure
As all we known intake of more than daily recommendation of salt for a prolong period of time causes high blood pressure resulting in lessening the function of our in regulating the blood flow in the arteries triggering the abnormal function of cells in the nervous system that increases the risk of nervous disorder including mood swing, anxiety and physical and emotional stress for some women with PMS.

F. Smoking
1. Hormone imbalance
Smoking besides increase the risk of lung cancer, it also depletes the levels of good estrogen and exhibit the levels of bad estrogen causing hormone imbalance resulting in increasing the risk of pre menstrual syndrome for some smoking women.

2. Immune system
Since cadmium is very toxins, it has an ability to attach to arteries wall and make the wall thinker that decreases the function of blood in transportation of nutrients and oxygen to our body cells need resulting in decreasing the body in hormone manufacturing that leads to hormone imbalance including pre menstrual syndrome.

3. Toxin accumulation
Cigarette contain high levels of toxic chemical compound that may danger to your health including increasing nervous tension and weakening the immune system such as negative allergic reaction and energy depletion.

4. Mineral deficiency
Cigarette contain high levels of cadmium which inhibits the mineral zinc causing minerals deficiency leading to symptoms of PMS.

5. Vitamin deficiency
Vitamin C and E besides is important for our body in fighting against forming of free radical and foreign invasion, it also helps to increase immune system in allergic reaction caused by hormone imbalance during menstrual. Smoking reduces the levels of vitamin C and E resulting in weakening the immune system and increasing the risk of negative allergic reaction leading to symptoms of PMS.

Premenstrual Syndrome (PMS) - The Effects of Fatty acids and Phytoestrogen


A. Fatty acids
I. Liver
a) Over consumption of saturated fat reduces function of liver in secreting bile into bloodstream and interference with liver in regulating function of pancreas in insulin production resulting in symptoms of PMS including food and sugar craving.
II.
Prostaglandins hormones

1. Definition
Prostaglandins are found in most tissues and organs. They are produced by all nucleated cells except lymphocytes. Prostaglandins are generated from
a) Gamma linoneic acid ( GLA)
b) Arachidinic acid (AA)
c) Eicosapentaenoic acid (EPA)

2. How prostaglandins effect women with PMS
a. Gamma linoneic acid ( GLA)
GLA is also known as essential Omega 6 fatty acid which has anti-inflammatory properties found primarily in vegetable oils. It may also be converted from linolenic acid before converting to prostaglandins PGE1. If there are blockage caused abnormal function of fat and protein metabolism, imbalance of essential Omega 3 and 6 fatty acids and nutrients deficiency, PGE1 may become bad prostaglandins resulting in symptoms of PMS including menstrual pain and cramps.

b. Arachidinic acid (AA)
Arachidinic acid (AA) is also converted from linoleic acids after converting to dihomogamma-linolenic acid (DGLA). It is produced as resulting of unbalance of Omega 3 and 6 fatty acids. Over production of PGE2 causes symptoms of PMS including menstrual pain and cramps. c. Eicosapentaenoic acid (EPA)

A. Migraine headache

1. Serotonin
Serotonin besides secreted by the pituitary gland when we come to stress, it is also secreted during migraine attack. Ayurveda treatments are individualized based upon a person’s imbalance of 3 doshas. The three doshas are Vata, pitta and kapha. Since ayurveda recognizes the imbalance of 3 doshas which cause pre menstrual syndrome, therefore if PMS is caused by
I. Vata imbalance
